Assume a front stance. With the back leg you sweep through your partner's front low leg. Without putting that leg down you then bring the leg up and hook kick the side of the opponent's head.

Front sweep (ashi barai):

Hook Kick (ura mawashi-geri):

Just practice each separately for a while. Then, when you're comfortable with them, just put them together.

It's more about timing than anything.

there are a couple of different ways you can combine/chain the two techniques together.

1.using lead leg only: sweep with the lead foot, at the end of its arc you continue the motion and hook kick to the head.

2. using back leg: sweep with your rear foot, when it lands you spin and hook-kick with other foot.

its really all about setting up your opponent. the ashi barai is really used to misdirect their attention.

Another fancy combo i've seen people do is an ura mawashi and then transition right into an osoto gari... I've seen it work quite a few times however i've seen people get reversed on the osoto gari as well (fail at it and ended up having it done to them instead)
