I am possibly going to be travelling with 5 or, 6 Juniors to a Karate seminar later this month, without their parents.

Could any instructor please offer me any advice as to what forms I need to use, such as parental consent forms ?

Do I also need to provide a risk assessment document for the parents ?

We never take part in any competitions so, this is the first time I might be travelling with Juniors and any advice would be very welcome.

I would definitely have a consent and release form signed by the parents! It should include a liability clause, consent for emergency treatment, and any pertinent contact and/or medical information. It may also be a good idea to have another adult traveling with you and the kids at all times.

I agree with having another adult. Maybe a parent volunteer? Otherwise I think a simple permision slip would do.
